WORKERS COMPENSATION

IBJI is willing to accept worker's compensation claims.  Our office requires the following information prior to scheduling the patients appointment... A letter from the work comp ins or employer, addressed to IBJI-Libertyville with the patients name, DOB, SS#, Claim #, contact person and a billing address of where to send the claims.  This letter may be faxed to our office at 847-234-2090.  Please be aware no work comp appointments will be made without this letter.  Our office is not willing to bill your personal health insurance is cases of workman's compensation.  If your case has not been determined as workman's compensation initially but there may be a possibility of being a workman's comp. claim, you may choose to self-pay for the appointment and submit the claim later for reimbursement.  If you have any questions regarding this, please feel free to call our office.
